Invoice Printing as a Statement

Until the last two invoices, all invoices have printed correctly. Now, I select print and the resultant product is a statement. Not an invoice. What box have checked incorrectly?

smart business pack

Just purchased smart business pack from staples. When i type in the code says not activated. is there anyway to activate this without going back to staples as it is quite a distance from me. Thanks.?
